CareBridge Corporate Office

Carebridge was founded in 2019 by Brad Smith and former Tennessee Senator Bill Frist. This pair of friends wanted to tackle the biggest challenge for those on Medicare and Medicaid; delivering better care for the money.

CareBridge uses a two-way tablet that sends messages to caregivers from patients. This tablet also monitors health signals and connects the patient with appropriate services, 24 hours a day. 

CareBridge’s impressive performance attracted significant attention, leading to participation from the owners of four major managed Medicaid plans in its $140 million Series B funding round last year. Elevance Health (formerly Anthem), UnitedHealthcare, Centene, and Aetna CVS all contributed to the funding, contributing to CareBridge’s valuation surpassing $2 billion.

These prominent health insurers, often referred to as payers, not only invested in the company but also emerged as its primary customers. Leveraging their support, CareBridge is expanding its presence into 30 states where it has secured Medicaid contracts.

Gentiva Health Services was founded on August 6, 1999, when Olsten Corporation split off its healthcare assets to form an independent, public company.

The company has grown substantially over the years, mainly through acquisitions. In 2010, it had acquired First HomeCare of Houston, Heritage Home Care Services, The Healthfield Group, Gilbert’s Home Health and Hospice, Physicians Home Health Care, and Hospice of Charleston. 

Gentiva Health Services is a provider of home health care, hospice, and related services in the United States. 

Gentiva provides health services to more than half a million patients annually through over 420 U.S. locations in 40 states.

In October 2022, Kindred Hospice of Midland changed its name to Gentiva after being acquired by the company.

In February 2023, the company acquired Heartland Hospice and Home Care from ProMedica.

The Hospice division of Gentiva also offers individualized programs that include a dementia specialty program, a cancer specialty program, a cardiac specialty program, and a pulmonary specialty program.

CareOne provides post-acute nursing and assisted living facilities. Currently, the company has 55 locations, mainly in New Jersey and Massachusetts.

In New Jersey, CareOne has 35 facilities, making it the largest assisted living business in the state.

CareOne states that it admits and discharges approximately 20,000 patients each year.

The company has also been nationally recognized for several successful fundraising efforts, including the Queen of Heart Ball for breast cancer research and the Valerie Fund, which benefits families of children with blood cancer. To date, CareOne LLC has raised over $5 million in philanthropic efforts.

In November 2019, CareOne was ordered to pay a former nursing home administrator $6 million after a Bergen County jury found that the African-American woman was fired because of her race.  

In April 2023, a former employee claimed she was fired after she complained about sexual harassment at the hands of her manager.

Life Care Centers of America Corporate Office

Life Care Centers of America was founded in 1970 by Forest Preston, who remains the sole owner and CEO.

Life Care is the third largest privately held, long-term elder care company in the USA. Currently, there are 256 locations in 28 states.

In 2016 Life Care agreed to pay $145 Million to resolve allegations of over-billing the government for unnecessary or unreasonable rehabilitation services in violation of the False Claims Act.

Between January 2006 and February 2013, Life Care submitted fraudulent claims for rehabilitation therapy by engaging in a systematic effort to increase its Medicare and Tricare billings. This was the largest settlement by a skilled nursing facility in DOJ history.

A Life Care Center facility located in Kirkland, Washington, was the source of a major outbreak of the Covid-19 virus when first reported on February 19, 2020, which became the official first outbreak in a United States nursing home.

In May 2023, a jury found that Life Care Centers were not responsible for the deaths of two patients due to Covid.

In April 2023, Newsweek rated 16 Life Care Centers as Best Nursing Homes in the USA.

Concentra Corporate Office

Concentra was founded in Amarillo, Texas, by three physicians who opened an occupational health care center in 1979. The company expanded to a second location in 1985.

The original name was Occucenter and in 2001, after merging with two other healthcare companies, all locations were rebranded as Concentra.

The company began its urgent care project in 2007 in response to the rising need for a cost-effective alternative to hospital emergency rooms for non-emergency ailments.

More than 30,000 patients visit Concentra’s medical centers every single day. According to the Bureau of Labor Statistics, Concentra treats one out of every seven occupational injuries in the United States.

In 2009, workers sued WalMart, CMI, and Concentra for “interfer[ing] with and limit[ing] the independent judgment of certain medical providers who treated injured workers employed by [WalMart] in Colorado who sustained on-the-job injuries”.

Concentra tried to have the suit dropped and fought the charges for three years. Judge Robert Blackburn approved a settlement of $8 million, requiring Concentra to pay $4 million, through its insurer, and Wal-Mart and CMI to pay the other half. Concentra was ordered to pay $520 to each Walmart employee that had been treated at one of its offices and $50 to employees who were treated at other clinics.

It was recently revealed that Concentra workers must give the company 4 months’ notice before leaving or quitting. If they fail to do so, they must pay the company in order to leave.
